whose primary role is to support student learning.

IMMUNIZATIONS:
The state of Missouri requires that all students be up-to-date with their immunizations. Please don't assume that we have your child's updated immunizations each year. If you have received a letter from our office requesting immunization records, please don't disregard it. It was sent because we need documentation from your doctor that your child's immunizations are up to date. Instruct the doctor's office to fax the immunization records to 636.225.4518, drop off a copy in person or have your child drop it off at the nurse's office. If the requested immunizations have not yet been given, we require the date and time of the next appointment in which they will be given. The appointment must be within a reasonable amount of time in order for your child's immunizations to be considered "in-progress". http://health.mo.gov/living/wellness/immunizations/index.php SCHOOL HEARING & VISION SCREENINGS: Each fall we perform vision and hearing screenings on all Pre-School, Kindergarten, 1st, 3rd, 5th, 7th, 9th & 11th grade students. If your child does not bring home a paper with the results, they will be mailed to your home. If it is indicated that their vision or hearing did not pass, please take your child for a follow-up visit to the proper professional. After they have been examined by an opthalmologist/hearing specialist, please forward the results to our office. There are many vision and hearing issues that can be resolved at an early age that can otherwise effect their learning success at school. POLICY FOR DISPENSING MEDICATION AT SCHOOL: If a student needs to take any medication at school, the following policy must be followed: 1) Prescription medication: Medication must be brought to school in a container appropriately and currently labeled by the pharmacy or physician. The label must include student's name, appropriate dosage, and time medication is to be given. Ask your pharmacist for a separate container with a prescription label which can be left at school. No student is allowed to carry any medication with the exception of an inhaler, and the inhaler may only be carried if a prescription from the doctor and parent's authorization is on file in the Nurse's office. 2) Over-the-counter medication: Any over-the-counter medication such as Tylenol, cough syrup, cold tablets, etc. will be administered at school only if accompanied by a physician's order. Please contact your physician and they may fax the information to our office at 636.225.4518. The note (RX) must include disbursement instructions verifying that this is to be given at school. No student is permitted to carry any over the counter medication. |
